DESCRIPTION

The Registered Nurse is responsible for the administration of direct and indirect client care for the Program using the nursing process and nursing judgments based on scientific principles. The R.N. is guided by the Nurse Practitioner Act and established policies and procedures for the unit.

DUTIES

1.0Observes and assesses client's course of treatment while in residence.
1.1Nursing assessments are performed in accordance with the Agency's protocol for conducting nursing assessments and its procedures relative to frequency and timeliness for the occurrence of this activity.
1.2Assessment data is organized, accurate and complete.
1.3Administers nursing care/treatment according to Agency protocol and procedures and in accordance with accepted nursing care standards and client's treatment plan.
1.4Recognizes signs of aggressive behavior and appropriately utilizes verbal and physical intervention techniques (NAPPI) to re-direct and prompt clients to safe and appropriate behavior.
1.5Utilizes client seclusion and/or restraints in accordance with unit protocol and procedure.

2.0Administers physician ordered medications to clients.
2.1Demonstrates the ability to administer medication in accordance with unit policy and procedure.

  • Documents medication education.
  • Accurately counts, dispenses and charts all medications given according to established procedure.
  • Administers all prn medications according to appropriate indications and verifies with original order
  • Administers all scheduled medications within 60 minutes of time ordered.
2.2Monitors and documents responses/reactions to medication in accordance with unit protocol and procedures.
2.3Maintains up-to-date knowledge of psychotropic medications and their side effects.

3.0Ensures and accepts responsibility for security of medication at all times.
3.1Medication count is conducted at the beginning and end of each shift.
3.2Reports and documents all medication variations on medication variance form and forwards to Program Manager.
3.3Medication room is organized, neat and orderly at all times.
3.4Medication room is secured at all times.

4.0Prepares written/EMR documentation for all services rendered.
4.1Written/EMR documentation for services rendered is performed in accordance with the Agency's standards for service documentation in the individual's medical record and according to individual's individual treatment plan.
4.2Accurately documents observations, procedures and care provided during the individual's course of treatment.
4.3Documents individual progress according to identified needs and goals of the treatment plan.
4.4All charting is signed with first initial, last name and title 100% of the time.
4.5Documentation is neat, legible and in accordance with accepted charting standards.

5.0Provides general direction and oversight to RSTs on assigned shift.
5.1Ensures that all RSTs have a clear definition of their work responsibilities.
5.2Consistently assigns tasks to RSTs in accordance with their personal abilities and limitations.
5.3Consistently demonstrates the ability to train and develop RSTs in their responsibilities.
5.4Ensures that all RSTs routinely utilize policy and procedure manuals and other available reference materials as necessary to consistently ensure quality of service provided.
5.5Consistently motivates RSTs in a positive manner; develops teamwork in accepting and practicing the policies and procedures of the Agency.
5.6Provides direction as necessary; always leads by professional example.
5.7Ensures client admissions are performed according to unit protocol and procedure.

6.0Establishes and creates a therapeutic milieu on assigned shift.
6.1Utilizes appropriate voice tone, volume and word choice when dealing with escalating clients.
6.2Demonstrates good problem solving techniques.
6.3Ensures groups are held as scheduled and directs that additional groups be held as clinically indicated.
6.4Provides feedback to RSTs and CAs regarding therapeutic interventions offering suggestions as needed.
6.5Demonstrates ability to assess when early interventions are needed and either provides the intervention or directs other staff.
6.6Acts as a role model in the use of de-escalation techniques and the teaching of adaptive coping skills with clients.

7.0Performs other duties as requested.
7.1Miscellaneous duties are performed according to instruction.

QUALIFICATIONS
