Source: FMS.EDUBelow is the list of documents which need to be sent to FMS Registrar office:The FMS application form printout taken should be affixed with the candidate’s photograph and duly signed . 
The documents of proof for reserved category (if any, refer below links to download the caste certificate proforma for FMS). 
The Demand Draft (if payment is not made online and made through DD) .
